Facility Portal Terms

Pending owner/counsel review: this is a complete draft matching how the facility portal actually works today, written to close a gap flagged in the project's own tracking. It has not been reviewed by an attorney and does not replace whatever separate service agreement your facility may already have with J&M Logistics for billing or scope of service — it governs use of the online facility portal specifically.

These Facility Portal Terms apply to any staff member who logs into J&M Logistics' facility portal on behalf of a healthcare facility, care facility, or similar organization ("the Facility") to request non-emergency transportation for a member, resident, or patient. They apply in addition to our general Terms and Conditions and Privacy Policy.

1. Facility Accounts Are Admin-Provisioned

Facility portal accounts aren't self-service — J&M staff set up your facility's account and each individual staff login after verifying the request. Each person who uses the portal should have their own login; logins should not be shared between staff members. The Facility is responsible for promptly telling us when a staff member should be removed (for example, when they leave the organization).

2. Booking on Behalf of a Member or Patient

When Facility staff submit a transportation request, they are doing so on behalf of the named passenger, not for themselves. By submitting a request, the Facility confirms that it has appropriate authorization to share that person's name, pickup/drop-off information, appointment details, and any mobility or accommodation needs with J&M for the purpose of arranging that transportation — and that the information submitted is accurate to the best of the Facility's knowledge.

3. Shared Visibility Within Your Facility

Every staff member logged in under your Facility's account can see that Facility's own booking history — this is intentional, so your team can coordinate coverage without one person being a single point of failure — but never another facility's bookings. If your Facility needs a staff member restricted from seeing certain bookings, contact us; the portal does not currently support per-booking visibility restrictions within one facility.

4. Facility Responsibilities

The Facility is responsible for: providing accurate pickup, destination, appointment, and mobility information for each trip; having the member/patient (or their authorized representative) ready at the scheduled pickup time; promptly notifying J&M of cancellations, consistent with our Cancellation Policy; and keeping staff portal access current, as described above.

5. Payment & Billing

Payment and billing arrangements for facility-booked trips (per-trip online payment, invoicing, or another arrangement) are as agreed between the Facility and J&M outside the portal itself, unless the Facility uses the portal's own online payment option for a specific trip, in which case our general Payment Terms apply to that payment.

6. Suspension of Access

We may suspend or deactivate a Facility's portal access, or an individual staff login, if we believe it's being misused, if information submitted through it is repeatedly inaccurate, or at the Facility's own request.

7. Changes to These Terms

We reserve the right to modify these terms at any time. Changes will be posted on this page (with the version date at the top updated); continued use of the facility portal after a change is posted constitutes acceptance of the updated terms.
